Clinical History
58-year-old female with palpable left breast mass detected on screening mammography. Core needle biopsy recommended. No family history of breast cancer. Patient reports mass present for approximately 3 months.
Specimen
A. Left breast, 10 o'clock position, core needle biopsy
Gross Description
Received in formalin labeled with patient name and "left breast 10 o'clock" are three cores of tan-pink tissue measuring 1.2 cm in aggregate length and 0.1 cm in diameter. Entirely submitted in cassette A1.
Microscopic Description
Sections show breast tissue with fragments of fibroadipose tissue and occasional ducts. The ducts show focal epithelial hyperplasia with some areas of increased cellularity. Scattered microcalcifications are present. The stroma shows mild chronic inflammation. No definite stromal invasion is identified in the limited tissue sampling.
Immunohistochemistry
| Marker | Result | Interpretation |
|---|---|---|
| Estrogen Receptor (ER) | Positive (weak, 30%) | Low-level expression |
| Progesterone Receptor (PR) | Negative | No expression detected |
| Ki-67 | 15% | Moderate proliferative index |
DIAGNOSIS
A. Left breast, 10 o'clock, core needle biopsy:
- Fibrocystic changes with focal epithelial hyperplasia
- Microcalcifications, benign type
- No evidence of malignancy in tissue examined
Comment
The findings are consistent with benign fibrocystic changes commonly seen in premenopausal and perimenopausal women. The focal epithelial hyperplasia shows usual type without atypia. Clinical and radiological correlation is recommended. Follow-up imaging per standard guidelines is suggested.
